Children entering kindergarten must be five years old on or before December 2. At the time of enrollment, the following requirements must be presented: A birth certificate or other birth record, such as baptismal certificate; Child's social security card or number; Proof of residence, which includes a PG&E bill, escrow instructions (in some cases), renter's agreement or a mortgage statement; California Immunization Record with evidence of up-to-date Polio, DTP (Diphtheria-Tetanus-Pertussis), MMR (measles, mumps, rubella) and Varicella (Chickenpox vaccine). The Hepatitis B series (series of 3 shots over a 6-month period) is required for students entering kindergarten and seventh grade; Evidence of Mantoux intradermal skin test (tuberculosis screening) must be presented.
